All County Home Health and Hospice - Hospice Per Visit Registered Nurse (Full-Time and PRN Available)

Job Summary:

The Hospice Registered Nurse (RN) delivers compassionate end-of-life care through patient assessment, symptom management, and family support. The RN coordinates closely with the interdisciplinary team to ensure comfort, dignity, and high-quality hospice care in accordance with patient goals and hospice standards.

Duties & Responsibilities:

  • Provide compassionate, patient-centered end-of-life nursing care in homes, facilities, or inpatient hospice units
  • Perform comprehensive patient assessments, including physical, emotional, psychosocial, and spiritual needs
  • Develop, implement, and update individualized hospice care plans in collaboration with the interdisciplinary team
  • Manage symptoms such as pain, dyspnea, anxiety, nausea, and agitation using evidence-based practices
  • Administer medications and treatments safely (oral, IV, SQ, transdermal, etc.)
  • Educate patients and families about disease progression, comfort measures, care techniques, and hospice philosophy
  • Identify discharge planning needs and implement prior to patient discharge

Communication :

  • Prepare clinical notes and update the primary physician when necessary
  • Communicate with the physician regarding the patient's needs and report any changes in the patient's condition
  • Coordinate care plan with community health-related persons

Additional Duties:

  • Participate in quality assessment and performance improvement programs
  • Participate in on-call duties as defined by the on-call policy
  • Ensure availability of necessary equipment and services
  • Instruct, supervise, and evaluate home health aide care provided every two weeks
  • Supervise Licensed Practical Nurses (LPNs)/Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed
  • Communicate regularly with physicians, social workers, chaplains, and aides to coordinate holistic care
  • Provide emotional support, anticipatory guidance, and grief education to patients and families
  • Document patient status, interventions, and outcomes according to hospice and regulatory requirements
  • Respond to urgent patient needs, perform on-call duties, and triage after-hours concerns
  • Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives
  • Train, supervise, or support hospice aides and other care team members as needed

Job Requirements:

  • Graduated from an approved school of nursing and licensed in the state of practice
  • Minimum of two years of experience, with preference for hospice care nursing
  • Maintains a current CPR certification
  • Licensed driver with an insured automobile in good working order
  • Self-directed and able to work with minimal supervision
  • Excellent observation, problem-solving, and communication skills
  • Ability to organize and prioritize workload independently
  • No management experience required
  • Able to perform required nursing skills
