Job Description

On-Call Registered Nurse: The continuity patients count on.

The Care Team Hospice is seeking an On-Call Registered Nurse to provide after-hours clinical support and urgent in-person care for hospice patients in home and facility settings. In this role, you’ll be a critical resource for patients, families, and care teams—ensuring comfort, safety, and seamless care when it matters most.

This is an ideal opportunity for an experienced RN who values independent decision-making, meaningful patient impact, and a predictable schedule.

Schedule, Hours & Territory:

  • Dedicated 7 days on / 7 days off rotating schedule.
  • On-call coverage from 4:30 PM to 8:30 AM.
  • Assigned coverage area determined by branch needs and patient census.

What You’ll Do:

As an On-Call RN, you will:

  • Provide expert clinical guidance, triage, and symptom management after hours.
  • Complete urgent in-person visits and admissions when needed.
  • Support patients and families through changes in condition with confidence and compassion.
  • Coordinate care with interdisciplinary team members to ensure continuity.
  • Accurately and timely document assessments, interventions, and communications in the EMR, HCHB (Homecare Homebase).
  • Serve as a trusted clinical resource during evenings, nights, weekends, and holidays.

You’re a calm, confident clinician who thrives in independent practice while staying connected to a supportive team.

Qualifications include:

  • Active Registered Nurse (RN) license.
  • Hospice field experience preferred.
  • Strong clinical judgment and decision-making skills.
  • Comfort providing guidance remotely and in the field.
  • Experience with HCHB (Homecare Homebase) preferred.

Field Role Requirements:

  • Valid driver’s license and reliable transportation.
  • Ability to pass criminal background and Motor Vehicle Record (MVR) checks.
