School-Based Physical Therapist (PT)
Location: Avondale, PA
Model: Full-time or Part-Time | On-site

Therapy Solutions Collective (TSCO) is looking for dedicated and motivated Physical Therapists to join our team for the 2025–2026 school year. We partner with school districts and medical facilities across the country, providing opportunities for Physical Therapists in a variety of settings. What You’ll Be Doing*:

• Conduct comprehensive in-home physical assessments to evaluate patients’ strength, mobility, balance, endurance, and functional abilities.
• Develop and implement individualized treatment plans tailored to each patient’s medical needs, home environment, and personal goals.
• Provide direct physical therapy services focused on improving mobility, safety, independence, pain management, and overall functional performance.
• Educate patients and caregivers on home exercise programs, safety techniques, equipment use, and strategies to support progress between visits.
• Collaborate with physicians, nurses, occupational therapists, and other care team members to ensure coordinated and effective treatment.
• Monitor and document patient progress, response to interventions, and changes in condition in compliance with regulatory and agency requirements.
• Recommend adaptive equipment or home modifications to increase patient safety and independence.
• Stay current with best practices and advancements in adult and geriatric physical therapy.

What We’re Looking For:

• Master’s degree or higher in Physical Therapy from an accredited program.
• Active state Physical Therapy license in the state where services will be provided.
• Experience working in a home-health based setting
• Strong collaboration and communication skills.
• Ability to adapt therapy interventions to meet the needs of students in a dynamic school environment.
• Someone who is organized, detail-oriented, and dedicated to providing high-quality therapy support.
• Someone who values collaboration and student-centered services.
